# Integrated code-review receipt

Scope: base `6ce7648afa0b69d17b765c1bb20a6fe7c3fa061f`, branch `fix/insights-verified-correctness`, all task-related tracked and new files. The pre-existing review artifacts are preserved.

Intent: close F01–F14 with explicit source permission, correct answer values, reliable jobs, honest provenance, earlier rows, responsive accessible UI, and measured evidence. User instructions prohibit commits, pushes, issues, and application deployment.

The three requested gpt-6-astra / xhigh workers reviewed the integrated changes. They covered correctness, security, API contracts, tests, lifecycle, migrations, performance, UI behavior, and accessibility. Simplification covered reuse, quality, and efficiency. The user-specified team replaced the skill's larger default roster and cross-model route.

## Applied findings

| # | Finding | Result |
|---|---|---|
| 1 | P1: Trusted identity lookup bypasses configured column policies | Actual-target permission, validation, masking, and final validation now run before raw auxiliary driver work. Dedicated real SQLite policy tests and driver compatibility gate: 67 passed. |
| 2 | P1: Completed trace spans leak source admission slots | Safe updates after trace completion. Cleanup includes tracing errors. Two regression cases pass. |
| 3 | P2: Migrated request keys permit duplicate execution | Matching legacy user/vault/conversation keys return 409 without dispatch. Other conversations retain independent keys. Regression passes. |
| 4 | P2: Federation admission wait disappears from tracing | The measured outer wait is attributed to the returned leg trace. The held-source regression passes. |

All changes remain uncommitted. Authorization, policy, contract, and concurrency tests are synthetic; production operation is still unverified.

The query review also drove fixes for approved CT request forwarding, primary and redirected source authorization, scope-aware masking, ordinal membership preservation, quoted identities, and exact-source cohort materialization. The UI review drove lost-ack recovery, visible generation-confidence wording, and currency-neutral coverage text. See the domain review files for those intermediate challenges and their tests.

## Requirements completeness

All F01–F14 have implementation and focused verification evidence. The parent verification report owns full-suite results and unavailable live checks. Whole-question interpretation, production grants/providers, multi-host rollout, and screen-reader behavior are not certified by this review.

## Coverage

Three reviewer artifacts were collected. The mechanics helper accepted every return: zero malformed findings, zero suppressed findings, and no exact duplicates. The identity-policy finding was reclassified from pre-existing to in-scope because the requested F01/F02/F13 work explicitly depends on preserving configured policies through auxiliary lookups.

A single validation batch inspected all four numbered findings. All four were validated. The UI reviewer originated none of these findings. This was same-model validation within the requested team, not cross-model corroboration. No validator was skipped on an independence claim.

See `validator.json`, `mechanical-findings.json`, and the original per-reviewer JSON files. Current reviewers report no unresolved actionable finding in their reviewed scope. Tests cannot prove every possible SQL, natural-language interpretation, browser state, or production topology.

## Verdict

Code review complete. All four formal findings were fixed and passed focused verification. Broader integration results remain in the parent verification report. Production readiness remains subject to the documented live checks.

Actionable findings: none from this review after the applied fixes. No merge or deployment was performed.

## Broader gate disposition

The first full suite returned 27,515 passed, 48 failed, and 21 skipped. All 27 new fixture-contract failures were repaired. The combined changed-test gate passed 735 tests. The other 21 failures reproduce on the base revision with matching IDs and reasons. The parent report records their scope and unresolved status. Later additive measurement and browser-fixture changes receive their own final checks before publication.

The final additive instrumentation gate passed 745 tests across all 36 changed Python test files in 71.87 seconds. A separate 142-test tracing/federation/merge gate passed. Parent wall time, nested check exclusion, child trace preservation, refusal and cancellation cleanup, and concurrent acknowledgement/persistence timing are covered. The lead inspected these final changes.
